Residency was the first time I experienced true exhaustion.
Not just being tired, but that deep, wired-and-drained feeling where everything feels off.
Night shifts were always the hardest for me. I could never fully adjust to sleeping during the day, and as the week went on, it compounded. Brain fog set in. My mood shifted. My patience was lower. And my appetite felt completely out of control.
I craved quick, easy foods. Carbs, sugar, fried food. The hospital cafeteria made that easy since those were often the only options available overnight.
At the time, I knew I felt better when I slept more, but I didn’t fully understand why everything else seemed to unravel when my sleep was off.
Now, I see it clearly in both the physiology and in my patients every day.
When Sleep Is Off, Everything Else Feels Harder
Patients will often say things like:
“I feel out of control with food.”
“I crave sugar at night.”
“I’m doing everything right but still hungry.”
And more often than not, when we zoom out, sleep is part of the picture.
Not always the only factor. But a significant one that’s often overlooked.
What’s important to understand is this:
This is not a willpower issue.
Your body is being driven differently.
What’s Actually Happening in Your Body
When sleep is disrupted, several key systems shift in ways that directly impact hunger, cravings, and metabolism.
Hunger and fullness signals change
Sleep deprivation increases ghrelin, the hormone that signals hunger, and can reduce leptin, which helps you feel full.
Even short-term sleep restriction in human studies has been shown to increase hunger and preference for calorie-dense foods.
Your brain seeks quick energy
When you’re tired, your brain is looking for the fastest source of fuel.
That’s why cravings tend to shift toward sugar, simple carbohydrates, and highly palatable foods.
This isn’t random. It’s adaptive.
Your body is trying to compensate for low energy.
Stress hormones increase
Poor sleep can elevate cortisol, which affects how we store fat, regulate blood sugar, and respond to stress.
Over time, this can contribute to increased abdominal fat storage, more blood sugar variability, and feeling more reactive or on edge.
Insulin sensitivity decreases
Even a few nights of poor sleep can reduce how efficiently your body handles glucose.
This can lead to more energy crashes, more cravings, and more difficulty maintaining stable energy throughout the day.
Why This Matters More Than People Realize
This is where I see patients get stuck.
They’re eating well, exercising, and taking their medications, but something still isn’t clicking.
Sleep is often the missing piece.
Especially in patients on GLP-1 medications, this becomes even more relevant.
These medications help regulate appetite, but they don’t override physiology completely.
When sleep is poor, I often see more breakthrough hunger, more cravings, and slower progress or plateaus.
Not because the medication stopped working, but because the body is under a different kind of stress.
Why You Crave Sugar When You’re Tired
When you’re sleep-deprived, your hunger signals are higher, your brain is looking for quick fuel, and your ability to make intentional choices is lower.
So you reach for foods that are fast, easy, and rewarding.
Your body is not failing you here.
It’s trying to protect you.
What To Do When Sleep Isn’t Perfect (Because It Won’t Always Be)
Even when we know sleep matters, there will be seasons where it’s not ideal.
Busy schedules, travel, stress, pregnancy, demanding careers. It happens.
The goal is not perfection. It’s knowing how to support your body when you’re not at your best.
On days after poor sleep, I focus on a few key anchors:
Prioritize protein early
This helps stabilize blood sugar and reduce the intensity of cravings later in the day.
Keep meals structured
Even simple structure prevents the cycle of skipping meals and overeating later.
Get light exposure early
Morning light helps regulate your circadian rhythm and improves sleep the next night.
Move your body, but adjust intensity
You don’t need an intense workout. Walking or light strength training is often enough.
Be mindful with caffeine
Helpful short term, but too much, especially later in the day, can perpetuate the cycle.
Avoid the “reset tomorrow” mindset
You don’t need to restrict or overcorrect. Consistency matters more than perfection.
Beyond Sleep Hygiene: What Actually Helps
Sleep hygiene matters, but when sleep issues become persistent, it’s usually not enough on its own.
This is where CBT-I (cognitive behavioral therapy for insomnia) becomes so important.
It focuses on retraining sleep patterns, addressing behaviors that reinforce insomnia, and shifting the thought patterns around sleep.
It remains one of the most effective, evidence-supported approaches for long-term sleep improvement.
A Thoughtful Approach to Sleep Aids and Supplements
When you’re exhausted, it’s completely understandable to want something that helps you feel better quickly.
And the truth is, some supplements can be incredibly helpful for the right person, especially when they’re used intentionally and alongside good sleep habits.
At the same time, I always want patients to understand what these tools can and cannot do.
Sleep is deeply connected to your nervous system, your circadian rhythm, and your daily patterns. So while supplements can support that process, they usually work best when they’re part of a bigger picture, not the entire solution.
Even melatonin, which is often viewed as a simple fix, doesn’t always work the way people expect. It can be helpful in certain situations, like jet lag or circadian rhythm shifts, but it’s not always the most effective long-term solution for ongoing sleep challenges.
It’s also important to remember that supplements are not regulated in the same way as prescription medications. I always recommend choosing brands that are third-party tested for quality and purity, and checking with your healthcare provider if you have any underlying conditions or are taking other medications to make sure there are no contraindications.
Magnesium (Glycinate vs L-threonate)
Magnesium plays an important role in calming the nervous system and supporting muscle relaxation.
Many people find it helpful, especially if they’re depleted or under a lot of stress. In practice, I often see it help people feel more relaxed in the evening and fall asleep more easily.
There are different forms, and they’re not all the same:
Magnesium glycinate
This is one of the most commonly used forms because it’s well absorbed and gentle on the stomach. It’s a good option for general relaxation and sleep support.
Magnesium L-threonate
This is a newer form that may cross into the brain more effectively. There is some early research suggesting it may support sleep quality and cognitive function, which makes it interesting, especially as we think about long-term brain health.
The research overall on magnesium and sleep is mixed, but clinically, it can be a helpful and low-risk place to start for many people.
Glycine
Glycine is an amino acid that plays a role in nervous system regulation.
There are small human studies showing that taking glycine before bed may improve subjective sleep quality and next-day fatigue.
It’s a simple, well-tolerated option that can be helpful for people who feel like their body has a hard time settling down at night.
L-theanine
L-theanine is another amino acid, commonly found in green tea, that promotes a state of calm without sedation.
Some studies suggest it may help reduce stress and improve sleep quality, particularly in people whose sleep is impacted by an overactive mind or anxiety.
It’s often used for that “wired but tired” feeling.
How I Think About It Clinically
I don’t think of supplements as a replacement for good sleep habits.
I think of them as supportive tools that can make the process easier.
For some people, they can be the difference between feeling wired at night vs relaxed, or struggling to fall asleep vs easing into it more naturally.
But the biggest and most lasting improvements still tend to come from consistent sleep patterns, light exposure, managing stress, and creating an environment that supports rest.
My Own Shift With Sleep
There was a time when I would push through exhaustion.
Now, I think about sleep differently.
Sometimes I choose sleep over an early workout.
Not because movement isn’t important, but because I understand that recovery, hormones, and energy are all connected.
That shift didn’t happen overnight.
It came from seeing what happens, both personally and clinically, when sleep is consistently compromised.
Travel, Disrupted Sleep, and Real Life
Travel is one of the biggest disruptors of sleep.
New environments
Different schedules
Alcohol
Late meals
Time changes
And when sleep is off, it doesn’t just affect how you feel.
It impacts hunger the next day, food choices, energy levels, and motivation to move.
Which is why creating an environment that supports better sleep, even while traveling, matters more than people realize.

Sleep isn’t just about rest.
It’s about recovery, balance, and resilience.
And when you understand what’s happening in your body, it becomes a lot easier to work with it instead of feeling like you’re constantly fighting against it.
